<p>Proof of address and documentation for all household members are required. </p><p>To qualify, your income must be below the following based on household size or be on government assistance like SNAP or WIC.</p><p>1) Proof of Address: Proof of address will be required at intake and yearly for reverification. Failure to provide proof of address may result in loss of service</p><p>Types of Acceptable Documents: • Driver’s License (less than a year)• Utility Bills• Bank Statement• Official letter• Lease agreement• Insurance documents• Employment documents• Car Registration </p><p>2) Proof of Household: Verification of household will be required at intake and yearly for reverification. Failure to provide proof of household will result in receiving food for one. </p><p>Types of Acceptable Documents: • Insurance cards• Medicaid cards• School documentation• Lease Agreement• Passports • Court documents</p>
