Facility address for SNAP: 247 N. MacDonald, Mesa - can call (or stop in at the main desk in the senior center) at any time during those hours to schedule an appointment (office visit or home visit) but they will most likely need to leave a message since the team often goes on home visits. There is not someone always in office to answer the team’s calls so we let participants know that their call will be returned within 48 hours
